The following are instructions on how to fill out and submit Wagoner Public Schools athletic participation forms:

1. The following information will be needed to complete the online forms:

Student’s ID number
Student’s full legal name
Date of birth
Email address
Student’s grade in school
Home phone number and cell phone number
Student’s current mailing address
Last school attended with school address
Primary physician and hospital
Emergency contacts with phone numbers

2. The required athletic participation forms are listed under the “Electronic Participations Forms” tab.
Click on a form name and fill out the information requested. If a question is not applicable, please answer with NA or UNKNOWN.

3. To sign the document, click inside the signature box and hold down the mouse button, which allows you to create an “electronic signature.”
If you make a mistake and need to start over, click on the refresh icon next to the signature box.
If you are using an iPad, you may create an electronic signature by holding your finger to the screen and signing or by using a stylus pen and signing.

4. Each form may be printed after all information is completed.

5. When the form is submitted, it will be reviewed by the Wagoner athletic trainer and a confirmation email will be sent to the email address that is provided at the bottom of each form
